Description

As a child care provider my objective is to nurture each child’s needs in a loving, safe and fun environment. Understanding and interacting with children is my passion. I identify with each child individuality and encourage children to believe in themselves. I relate to children by building a respectful relationship. Working to understand each child’s strength and some weaknesses to help enhance their self-esteem through play and routine schedules
Admission conditions
Tuition:
Weekly tuition is due in advance and is to be paid on Friday's at drop off. A late fee will apply of $5.00 per a day if not paid by pickup time on Friday's at 6pm. If your child is not picked up by 6pm a late charge of $1.00 a min per child will apply to your next payment of tuition.

Deposit:
A deposit $150.00week/$30.00per day is required to hold a spot for your child. The deposit will be applied toward the first week of care. If for any reason the parents/guardians decided not to have care with Raquel's Home Daycare this deposit will be NON-REFUNDABLE.


Vacation for Provider:
The provider has two-week’s paid vacation per year. Parents/Guardians will be notified in writing seven weeks in advance of the provider's vacation. Parent/Guardian is responsible for finding alternate care for and will be charged full price for provider's vacation time. If provider takes vacation within first 6months, that one week vacation will be a NO CHARGE to Parents/Guardian.

Absent/Late:
Please inform the provider at least one hour prior to arrival time if your child will be absent or late.

Personal day/sick days for Provider:
Provider is allowed 4 personal days/sick days, with pay, per year. Parents will be notified of provider's personal day/sick day. If I am sick I will let you know no later than 5:30am that day. Parents/Guardians will have the option to bring their child to childcare.

Holiday's Childcare will be CLOSED:
New Yrs Day, Memorial Day, July 4Th, Labor Day,
Thanksgiving Day & the day after (this a travel day)
Christmas Eve
Christmas Day

Jury Duty:
If the provider has to attend jury duty selection, this will be paid days off. If provider is selected for jury this will be paid no more than 3 days off for the length of the trial. Parents will be responsible for alternate care.

Bereavement:
The provider is allowed 3 days paid for immediate family members. The provider is allowed 1 day paid for any other family or friends. Parent/Guardian will be responsible for alternate care.

Bad Weather Days:
If there is bad weather or sudden severe weather that causes power outages and the provider is unable to keep a comfortable and controlled environment the childcare will be CLOSED. The childcare will notify parents/guardians that it will be closing until weather clears up.

Illness of a child:
Children must be kept at home if he/she has a fever of 100 degrees or higher. If the child shows signs of a communicable illness parent/guardians will be notified as soon as possible. If the child had green drainage it is considered contagious the child may not return to childcare until drainage is clear. If child gets ill during childcare the parents/guardians will be notified right away and will be picked up within the hour. Child may not return until fever is gone for 24hours.

Pets:
We are an animal free home.

Discipline:
Children will have positive guidance, redirection, and distraction for undesirable behavior. As a last resort when necessary the child will be placed in timeout chair.

Potty Training:
Parents/Guardians and provider must agree on potty training of which the time is right for the child to begin. When potty training starts the child will need to wear pull-ups. The parents/guardians must provide pull-ups and two changes of clothes per day.

Laying Position:
Infants that are under a year old will be placed on their backs to sleep. If they are old enough to turn over on their own they will still be laid on their backs also. If parents/guardians have written permission from a medical doctor stating that the infant may sleep in a different position.

Personal Items:
The provider will need a change of clothes to be left at the childcare. Please have change of clothes weather appropriate. Your child may bring his/her own stuffed animal to sleep with. The moment I hear the child(ren) awake I will be getting them out of their nap bed or nap mat.

Immunizations:
Immunizations are state required. Immunizations records must be updated each time the child has any immunizations done.

Medications:
State Law prohibits the provider to administer medication without a completed and signed medication form from parent/guardians. Medications must be in the original container, with the child’s name on the container. If child is taking medications, parents/guardians must have a for which the medication is for and dosage given.


Accident/Incidents:
Parents/Guardians will receive a report of any accidents/incidents that occur while your child is in the childcare home. Parents/Guardians will be required to sign the report. A copy of the report will be kept in the child's file and parents/guardians will receive a copy.

Medical Release:
A medical release must be signed. A statement of health must also accompany it, with original doctors signature.



Emergency Procedures:
There will be regular fire and tornado drills done each month. In case of tornado warning, we will be located in the guest bathroom and I will be notifying parents/guardians when bad weather comes up. The bathtub is the inside room in my home. If and emergency should happen 911 will be called. If a child or provider has to be transported to the hospital backup care will be provided until parents/guardians can be contacted by my backup care or provider to pickup children immediately.

Motor Vehicle Traveling:
Parents/Guardians understand that in an emergency the provider must transport the child(ren) only. I do not do any after school pickup. By signing the contract , you give consent for emergency transportation from childcare. When the child is in a motor vehicle, safety restraints will be worn. The vehicle must have insurance coverage and is required by state law and must be current. As long as these precautions are taken, this will relinquish the childcare provider from any liability in case of a motor vehicle accident.

Telephone:
I only have a cell phone. I do have a earpiece. If I do not answer right away when you call and my voice mail picks up, this means I am busy taking care of the children. When I am able I will return your call.

Personal Visits:
You are welcome to drop by at anytime.

Pool Release:
There is not a big pool in my backyard. We may play in a baby pool in the summer but never deep water.

Playground Equipment & Toys;
Parents/Guardians will be shown the toys that children will be playing with. There is a park not far from my house. Little tikes toys that are in the backyard.

Authorization to Pick Up Child(ren)
Parents/Guardians must have written documentation of who may pick-up the child(ren). That person must show a Valid Id to the provider to remove the child from childcare. Telephone calls will NOT be allowed to relinquish the child who's is not listed. In the event of divorce, the provider will not deny either parent the right to pick-up the child(ren) unless there is a copy of the custody order on file with the provider. The custody must state Custodial Guardianship and visitation rights to both parents. If an issue arises during childcare hours the provider will contact the police to handle the situation. If I feel that the child is in danger both parents will be contacted.


Removing Child(ren) from Childcare:___mother initial_____Father initial

This Contract may be terminated by the Parent/Guardian or the Provider by giving a two week written notice. Tuition for the two week written notice of termination is due at the time of notice given, even if your child does not continue to attend during that time. When removing the child from childcare, all tuition must be current, including the payment of the two week notice tuition. If parent/guardian fails to pay the two week tuition, the parent/guardian will be responsible for all legal fees, court fees, reimbursement for provider's time off work and other cost pertaining to the collection of tuition.

The Provider may immediately terminate this contract without notice for the following: If Parent/Guardian does not make payments when due: does not pickup an ill child when notified: if the child is a risk to the other children in care or disrupts the daily running of the childcare: if the provider sees any other need to do so.

The Signature of the Parent/Guardian to these Policies indicates you have read and agree to abide by the written contract and policies of the Provider. The parent/guardian understands that they are responsible for any and all court fees, attorney fees, and legal fees, missed day of pay fees for the provider to attend court or any other cost incurred. The Provider may change contract or policies at any given time, parent/guardian will be notified in writing of the changes.
